Follow the simple 5-Step application process to enroll in one of MSA's training sessions:

STEP 1. Choose your preferred session.
STEP 2. Complete the application form below.
STEP 3. Sign the Medical Release and Waiver Form below and 'submit' the forms.
STEP 4. MSA will contact you within a couple of days. Based on an assessment of skill level, desired goals, financial goals and schedule logistics, MSA will recommend a program (session type, player mix [if small group session], and number of sessions) specifically for your child.
STEP 5. Make Payment based on recommended program.

ASSUMPTION OF THE RISK, RELEASE OF LIABILITY AND INDEMNITY AGREEMENT

ASSUMPTION OF THE RISK. I, the undersigned, on behalf of myself and the applicant above ("Student"), understand that Student's participation in soccer training provided by Montoya Soccer Academy (the "Academy") involves exposure to risks inherent in the sport of soccer, including but not limited to, falling, collisions with other players, participants or objects, and injuries assoicated with vigorous participation in the sport. I further understand that these risks cannot be eliminated from soccer training or games that these risks can result in injury or possibly even death. On behalf of myself and Student, with the full knowledge of these risks, I hereby expressly assume all risks associated with Student's participation in soccer-related activities provided through the Academy.

RELEASE OF LIABILITY AND AGREEMENT TO INDEMNIFY. In consideration for the benefits received from Student's participation in soccer training through the Academy, I, the undersigned, on behalf of myself and Student, hereby forever release and discharge Montoya Soccer Academy, its employees, owners, and agents (collectively the "Released Parties"), from any and all liabilities or claims that I, my assignees, heirs, distributees, guardians, legal representatives and Student (collectively, the "Releasing Parites") now have or may hereafter have on account of injury, death, property loss or other damages resulting from negligence or other acts of the Released Parties, as a result of Student's participation in soccer-related activities. I further agree to defend and indemnify the Released Parties for any loss or damages realted to Student's participation in soccer-related activities through the Academy.

I have carefully read this "Assumption of the Risk, Release of Liability and Indemnity Agreement" (the "Agreement"), and fully understand its contents. I understand that this Agreement is a contact that is legally binding me, my assignees, heirs, distributees, guardians, and legal representatives and Student and that I am releasing rights to bring a legal action by accepting it.

After submitting your application form, MSA will contact you directly. After assessing your child's skill level and desired goals, and taking into account any financial considerations and schedule logistics, MSA will recommend a program consisting of session type, any matches with other peers and number of sessions. Once a progam is mutually agreed upon, your child will be registered for training.
